Supported Community Connections (SCC)
Supported Community Connections (SCC)
Strive care services LLC will provide 24/7 daily support to persons with intellectual and developmental disabilities (IDD). Strive care services LLC will provide IRSS services to individuals wanting to live as independently as possible in the community of their choice. IRSS is provided to 3 or fewer persons receiving services who live together in a personal care alternative (PCA) setting or a (Host Home) setting. IRSS uses a variety of living arrangements individually designed to meet each person’s needs. The individual may live in a home owned or leased by the agency, their own home/apartment or a Host Home.

Supported Community Connections (SCC)
Strive care services LLC will provide SCC services to individuals in the community. SCC services are designed to support individuals with opportunities to build relationships and natural supports in the community, interact with community members, promote safety and independence, and work on personal goals. Strive care services LLC will utilize typical community settings as part of the SCC learning environment. SCC services can be provided on a one-time basis or on a recurring basis. SCC services will be provided on a one-to-one basis or in groups between 2-5 persons.
SCC will provide individuals with experience accessing their community competently, confidently, and safely; expose individuals to a variety of cultural, social, recreational, volunteer, and employment opportunities available to them in their community; and, educate individuals about how to access their community in an informed manner. SCC will provide individuals the opportunity to plan, choose, and learn while accessing their community.
